A hammer toe can be defined as a condition that causes your toe to bend downward instead of pointing forward. While it can occur on any toe on your foot, it usually affects the second or third toe. If your baby toe curls instead of buckling, it is also considered a hammer toe. There are two types of hammer toes. If your toes still can move around at the joint, then it is considered a flexible hammer toe. It is a milder form of the condition and there are more treatment options. The other type is called a rigid Hammer toe, which occurs when the tendons in your toe become so rigid that they push your toe joint out of alignment, and it cannot move at all. Typically, you will need surgery to fix it.
Hammer toes are most frequently caused by a muscle – tendon imbalance in the foot, and are seen both in adults and children. Foot muscles work in pairs to Hammer toes straighten and bend the toes. If your foot has a biomechanical defect, the muscles tighten and the tendons shorten. Eventually, the toe muscles can?t straighten the toe, even when barefoot. Contributing factors are poor choices in footwear, arthritis, or trauma.
The most obvious symptom of hammer, claw or mallet toe is the abnormal toe position. This is usually combined with pain: the abnormal foot position leads to excessive friction on the toe as it rubs against any footwear which can be extremely painful. Corns & Calluses: repeated friction can result in the formation of a foot corn or callus on top of the toes. Stiffness, the joints become increasingly stiff. In the early stages, the toes can usually be straightened out passively using your hands, but if allowed to progress, the stiffness may be permanent.
Your doctor is very likely to be able to diagnose your hammertoe simply by examining your foot. Even before that, he or she will probably ask about your family and personal medical history and evaluate your gait as you walk and the types of shoes you wear. You’ll be asked about your symptoms, when they started and when they occur. You may also be asked to flex your toe so that your doctor can get an idea of your range of motion. He or she may order x-rays in order to better define your deformity.
Non Surgical Treatment
Your podiatrist may recommend one or more of these treatments to manage your hammer toes. Wear shoes with roomy toe boxes which don?t force your toes together. Exercises to stretch and strengthen muscles in the toes. Over the counter toe straps, cushions, and corn pads. Custom orthotic inserts. Toe caps or toe slings. In severe cases, surgery to release the muscles in the affected toes.
Surgery is the approach that is often necessary to correct hammertoe that fails to respond to nonsurgical management. Surgery is appropriate when the muscles and tendons involved in a hammertoe problem have become so tight that the joints are rigid, misaligned and unmovable. There are a number of surgical techniques for dealing with the complex range of joint, bone, muscle, tendon and ligament abnormalities that define each hammertoe’s make-up. To correct a hammertoe deformity, the surgeon’s goal is to restore the normal alignment of the toe joint, relieving the pressure that led to the hammertoe’s development (this should also relieve the pain, as well). To do this, he or she may remove part of the boney structure that creates a prominence at the top of the joint. Tighten or loosen the muscles, tendons and ligaments around the toe joints. Realign the toe bones by cutting one or more and shifting their position, realigning muscles, tendons and ligaments accordingly. Use screws, wires or plates to hold the joint surfaces together until they heal. Reconstruct a badly damaged joint or replace it with an artificial implant.
A bunion is a firm, painful bump that forms over a bony bulge at the base of the big toe. In most cases, the big toe joint also is enlarged and has degenerative arthritis. The toe also may be pushed toward the second toe (hallux valgus). Bunions tend to be inherited, but they also are common in the following groups. Women who wear high heels. People who wear shoes that are too narrow or too pointed. People with flatfeet. All of these situations force the big toe to drift toward the little toes, and this can cause bunions to form.
Bunions are most often caused by an inherited faulty mechanical structure of the foot. This faulty structure causes the drifting of the great toe and the bone to become prominent on the side of the foot. The skin then gets pinches by this bony prominence and the shoe. Therefore in most cases bunions are not caused by tight shoes but are made more painful by tight shoes. End stage bunions may become painful both in and out of shoes.
Symptomssymptoms and problems caused by bunions include pain. You may then have difficulty walking due to pain. Inflammation and swelling at the base of the toe. This sometimes becomes infected. The foot may become so wide that it can be difficult to find wide enough shoes. You may get arthritis in the big toe. The second toe can become deformed. In severe cases, the big toe can push your second toe up out of place.
When an x-ray of a bunion is taken, there is usually angulation between the first metatarsal bone and the bones of the big toe. There may also be angulation between the first and second metatarsal bones. These angular irregularities are the essence of most bunions. In general, surgery for bunions aims to correct such angular deformities.
Non Surgical Treatment
Padding with a number of different materials (eg felt) to reduce pressure on the painful prominence of the bunion. Physical therapy can be used to help with the symptoms and improve the range of motion (this is particularly helpful if the pain is coming from inside the joint, rather than from shoe pressure). Manipulation of the joint can be used to help with this (manipulation will never correct the alignment of the joint). Any corns and calluses that are causing symptoms should be treated. The correct fitting of footwear is essential for anyone who is serious about doing something about their bunions and hallux valgus. It may be possible to have your shoes stretched over the area of the bunion to also relieve pressure. Foot orthotics may be useful in helping with the instability about the joint. They may be more helpful if there are other symptoms in the foot as well, as their use in “treating” bunions is controversial. They may play a role in slowing progression and in the prevention of bunions developing again after surgical correction. Exercises can be important in maintaining the mobility of the joint in those with bunions, this is especially important for the arthritic type pains that may be originating from inside the joint and for the prevention of these painful symptoms in the future.
Procedures can range from shaving off excess bone to restructuring and fusing the big toe. For mild conditions, you may simply need the connective tissues holding your big toe to be tightened so they hold the digit in the correct position. More advanced bunions will need more manipulation and involved remedies. Cuts in the bone tissue can help our specialists realign the toe. You may need to have the damaged portion of the joint removed. In severe cases, the joint may be fused to prevent it from moving out of position again. If your bunion created other foot complications, like hammertoes, our specialists may correct those during the procedure as well.
Overpronation is a term that gets thrown around a lot by psuedo-experts and there is so much written online about it, that it can get very confusing to determine what is legitimate and what is not. Pronation is a normal motion of the foot that occurs when the ankle rolls inwards and the arch collapses. This is normal. Its not something evil. Overpronation is assumed to be when there is too much of it. There certainly is debate as to just how much is too much, as some people have large amounts and have no problems, other have small amounts and do have problems. It all comes down to individual differences and how much force is needed to stop the foot moving. The greater the force, regardless of the amounts of pronation, the more likely it is to be a problem. Some still like to debate if it is a problem or not. Some studies have shown that its not a problem and other studies have shown it is. If the data from all these studies are pooled, then the conclusion was that, yes, overpronation is a problem that was statistically significant, but it was only a small risk factor for problems. An associated finding of overpronation during a gait analysis is an abductory twist.
There may be several possible causes of over pronation. The condition may begin as early as birth. However, there are several more common explanations for the condition. First, wear and tear on the muscles throughout the foot, either from aging or repetitive strain, causes the muscles to weaken, thereby causing the foot to turn excessively inward. Also, standing or walking on high heels for an extended period of time also places strain and pressure on the foot which can weaken the tissue. Lastly, shoes play a very common factor in the development of over pronation. Shoes that fail to provide adequate support through the arch commonly lead to over pronation.
Common conditions seen with overpronation include heel pain or plantar fasciitis. Achilles tendonopathy. Hallus Valgus and/or bunions. Patellofemoral pain syndrome. Iliotibial band pain syndrome. Low back pain. Shin splints. Stress fractures in the foot or lower leg.
At some point you may find the pain to much or become frustrated. So what are you options? Chances are your overpronation has led to some type of injury if there’s pain. Your best bet is to consult with someone who knows feet. Start with your pediatrist, chiropodist or chiropractor. They’ll be able to diagnose and treat the injury and give you more specific direction to better support your feet. One common intervention is a custom foot orthotic. Giving greater structural support than a typical shoe these shoe inserts can dramatically reduce overpronation.
Non Surgical Treatment
If pronation is diagnosed before the age of five it can usually be treated in such a manner that the bones and joints will be aligned properly as growth continues. This may prevent the arch from collapsing, as well as allowing the muscles of the leg to enter the foot without twisting. With proper and early treatment, the foot will not turn out at the ankle, and the child?s gait will improve. Treatment for pronation in children may include: night braces, custom-made orthotics, and exercises. These treatments usually continue until growth is complete, and then the adult may need to wear custom-made orthotics to prevent the pronation from returning (the foot, as every other part of our body, tends to return to its original form if preventive measures are not taken). One side note: frequently, pediatricians will wait too long, hoping that the child will ?outgrow? the problem. By the time they realize that the child?s feet will not improve, it is too late to change the foot. In these cases, custom-made orthotics is used to prevent the pronation from becoming worse.
Many of the prevention methods for overpronation orthotics, for example, can be used interchangeably with treatment methods. If the overpronation is severe, you should seek medical attention from a podiatrist who can cast you for custom-made orthotics. Custom-made orthotics are more expensive, but they last longer and provide support, stability, and balance for the entire foot. You can also talk with a shoe specialist about running shoes that offer extra medial support and firm heel counters. Proper shoes can improve symptoms quickly and prevent them from recurring. Surgery can sometimes help cure and prevent this problem if you suffer from inherited or acquired pes planus deformity. Surgery typically involves stabilizing the bones to improve the foot?s support and function.
